In order to provide a fast and timely response to any issue, whether in or out of warranty, Radius would ask that all customers use the following fault report document and ensure it is attached to the returned item.
This way we can track the item and also identify the failure mode more quickly.

To make sure that all the functions work correctly when you open and save the fault report, you need to have Acrobat Reader 7.0.9 or an even newer version installed at your computer.
